AHRQ Report on CPAP Devices - Consider The Ramifications

There's a lot to take in here.  You'll need to read the entire article to unpack the findings.

According to the article:

"Spoiler alert: The report, Long-Term Health Outcomes in Obstructive Sleep Apnea: A Systematic Review of Comparative Studies Evaluating Positive Airway Pressure and the Validity of Breathing Measures as Surrogate Outcomes, does not answer the main clinical question it asks. Rather, for each outcome—cardiovascular disease, driving accidents, diabetes, mental health, cognitive function, quality of life, and many others—it details why the evidence is inadequate, over and over again."

AHRQ: Studies Fail to Show Long-Term Benefit for CPAP - MedPage Today

According to this article, AHRQ: Studies Fail to Show Long-Term Benefit for CPAP -  Sleep apnea specialists dumbfounded at critique of "gold standard" treatment:

"The big fear that I have, and many of my colleagues have, is that the way this report is worded could easily be misinterpreted as saying that prescribing CPAP doesn't improve these (long-term) outcomes, so we shouldn't be paying for it," said David Rapoport, MD, who directs the sleep medicine research program at the Icahn School of Medicine at Mount Sinai in New York City.

He said CPAP is widely recognized as the most effective treatment for improving OSA symptoms, such as snoring and daytime sleepiness.

"Nothing else even comes close," he said. "CPAP s really a remarkable treatment for addressing the breathing disorder associated with obstructive sleep apnea. But it has to be used, and compliance remains a big issue."

According to the article ...

"CMS in an interim final rule published March 30 appeared to significantly expand access to respiratory therapy by temporarily waiving the coverage criteria and face-to-face requirements during the coronavirus pandemic.

The agency says NCDs and LCDs that point to clinical indications for coverage for respiratory equipment— including but not limited to home oxygen, CPAPs, RADs, ventilators and infusion pumps—as well as to face-to-face evaluations, will not apply during the public health emergency."

A Scary New Trend: Six-Year Lookback Audits - HME Business

"The Office of Inspector General (OIG) regularly performs audits and reviews as part of its normal functions. In June 2018, it released a scathing report entitled, “Most Medicare Claims for Replacement Positive Airway Pressure Device Supplies Did Not Comply With Medicare Requirements.”"  ... 
"In layman’s terms, suppliers either have to review every claim they have submitted for PAP supplies for the previous six years, or they must identify the universe of claims by taking a statistically valid random sample and extrapolating the results of the audit once the sample has been reviewed. They are asking suppliers to do something that requires the high-level skills and knowledge of an expert statistician."

"Medicare, the government insurance program for seniors and the disabled, began requiring CPAP "compliance" after a boom in demand. Between 2001 and 2009, Medicare payments for individual sleep studies almost quadrupled to $235 million. Many of those studies led to a CPAP prescription. Under Medicare rules, patients must use the CPAP for four hours a night for at least 70 percent of the nights in any 30-day period within three months of getting the device. Medicare requires doctors to document the adherence and effectiveness of the therapy."

Unfortunately, CMS puts that burden onto the supplier and most physicians don’t know the documentation requirements. There should be some sort of internal QA or prior approval process. These days, it is not enough to just request documentation — you should also review it and determine whether the patient qualifies. Many suppliers are now being forced to turn away patients and transfer liability to them because they can’t get the documentation that they need to substantiate reimbursement."

"The federal government is seeking public input on whether it should require sleep tests for commercial drivers and railroad workers.

The U.S. Department of Transportation’s Federal Motor Carrier Safety Administration and Federal Railroad Administration announced the proposal March 8. The National Transportation Safety Board has recommended the DOT take action to address OSA screening."

"U.S. shipments of auto-positive airway pressure devices will jump a considerable 70 percent over 2013 levels by 2017, according to a new report from IHS Technology.

Shipments of Auto-PAP devices in the United States will increased at a compound annual growth rate of 14.2 percent over the next four years, reaching 1,312,811 units by 2017, according to the report, Sleep Diagnostics, Sleep Therapy and Sleep Interfaces - World - 2013. Auto-PAP shipments will surpass those of continuous positive airway pressure (CPAP) machines in 2015, the report also noted."

"MUST HAVE a EXEMPTEE HMDR , or a Pharmacist license, if providing the following items: Legend devices (RX only label), oxygen, TENS units, CPAP, Nutritional feeding kits (w/syringes), IV pumps. (Any item that has a caution (legend) that is only for sale by or by order of a physician. It is not an item that can be purchased over the counter but must be prescribed.) The EXEMPTEE or Pharmacist must be an employee and under full control of these items during dispensing of prescription devices."
